Annual comprehensive preventive health assessment and exam
- EKG (as clinically indicated).
- Immunization recommendations.
- Formulation of a personalized preventive medicine plan.
Additional Medical Care
- Office visits:
- Routine office testing and procedures;
- Sick visits;
- Blood pressure checks; and
- Weigh-ins.
- House calls and acute rehabilitation facility visits when clinically indicated:
- Consultation by phone with emergency room providers, as necessary and appropriate.
- Consultation by phone with attending hospital physician, as necessary and appropriate.
- Review of tests and consults from other providers as they become available.
Coordination of Care
- Referral to appropriate specialists and facilities.
- Assistance in scheduling appointments with other physicians and facilities for tests and consultations.
- Coordination of your care with specialists and facilities.
- Coordination and monitoring of hospital admissions, inpatient care, and rehabilitation care.
Administrative & Access
- Direct access to Dr. Kalikhman by cell phone or email (except during vacations or continuing medical education, where coverage will be provided by a qualified colleague).
- Completion of pre-authorization forms for medical services and prescriptions (except where in-plan physician referral is required).
- Copies of lab and other diagnostic test results by fax or email.
- Convenient appointment and prescription refill requests by phone, and email.
- Completion of medical history forms and medical necessity forms.
- Office announcements and reminders emailed directly to you.
- Total number of patients in the Practice limited in order to permit more personalized care and service.
- Wait times for scheduled visits of less than 15 minutes (except in cases of unanticipated patient emergency).
- Urgent office appointments available same day or next day.
